Capture the visit once, while the details are clear.
Use the print button below and choose A4 portrait. The printed version hides the website and keeps the complete blank docket together on one page.

General operational template only. Adapt it to your company process, customer terms and any work-specific contractual, quality, safety or record-keeping requirements. Customer acknowledgement records the visit only unless your own approved wording states otherwise.
How to use it
A useful docket separates facts, completed work and follow-up.
Agree who completes the record, when the office reviews it and what output the customer should receive. Keep photographs and readings attached to the same job rather than sending them separately.
This is not a certificate or proof that work is compliant.
It is a general administrative starting point. Have the people responsible for contracts, safety, quality and technical decisions approve the fields and wording used by your firm.
Identify the job clearly.
Use the customer, address and work-order number already recognised by the office.
Write what happened.
Separate the reported issue, work completed and findings.
Keep evidence with the visit.
Reference photographs, readings and documents before the technician leaves.
Give follow-up an owner.
A recommendation is useful only when somebody knows what happens next.
